In order to complete the Patient Care Technology Application you will need electronic copies of the following documents. Because the Patient Care Technology application form must be completed within a 45 minute time frame, applicants should scan documents and have available before you begin.
- PCT Program Statement of Understanding
- PCT Immunization Records form and copy of records
- Complete an AHA CPR for Healthcare Providers Course & Submit Copy of Card

- DPS background Check Form
If you do not have electronic copies of the above documents, you may submit forms to Department of Health Sciences Second Floor of the UHS building or Fax to: 903-434-4425.
Patient Care Technology Application
NOTE: Credit may be awarded toward courses NURA 1401 and NURA 1407 should the student possess a CURRENT certified nurse’s aide license. Students seeking credit for experience will need to make the appropriate request at the counseling center and make arrangements to document proficiency. The student is responsible for identifying specific experiences in which he/she seeks credit. There is a $40 fee for credit by experience placement.
